CHECK / GIFT CARD Minimum Withdrawal: $50.00 Your first withdrawal processed each month, regardless of method, will be free. Subsequent requests will be subject to a standard check-cutting fee of $40. All checks will be sent via a courier service. When you request a check you may receive a Gift Card instead, a Gift Card allows you instant access to your funds. You must deposit all checks into your bank account within 45 days of the delivery date. After this time, we cannot assume responsibility for any issue related to redeeming the check. |

Withdrawal Policy
Withdrawal policy updated October 27th, 2011.
In keeping with our KYC policy we require:
- Proof of ID: A copy of a valid form of ID (Driver's License or Passport)
- Proof of address: A copy of a recent utility bill showing your name and address
Your Passport or Driver's License may also serve as proof of address if all details are clear, if it has not expired and if the address on the ID matches the one on your account. If the address on your ID is different from the one on your account then we will require additional proof of address in the form of a recent utility bill in your name. The bill must be less than 6 months old.
We require you to provide this information *once *only. We will then securely file it for future withdrawals.
Please scan your documents, save them preferably as jpegs and upload them here to our secure server for review. Alternatively, you can take a digital photo with your camera or your phone and upload them in the same way. Please check the quality of your images to ensure that all pertinent information is legible. The attachment or attachments should not exceed 5MB in total and ideally should be much smaller.

In order to ensure that you are the only person who can withdraw funds from your HollywoodSportsbook.com account, you must enter your Personal Identification Number (PIN) before you can make a withdrawal.
